Abstract

Pulmonary tuberculosis (PTB) is an uncommon respiratory infection that rarely leads to the development of hyponatremia, a condition characterized by low levels of sodium in the blood. In most cases symptoms andiated hyponatremia are mild to moderate, often lacks noticeable symptoms, and can be effectively reversed through treatment with anti-TB medications. Hyponatremia is diagnosed when the serum sodium level falls below 135 mmol/L, and it is considered severe when the level drops below 125 mmol/L. Among electrolyte imbalances, hyponatremia is one of the most frequently observed, with a prevalence ranging from 1 to 4% in its severe manifestation. This journal reports the case of a male patient who presented with decreased consciousness, suspected to be associated with moderate hyponatremia, tuberculous meningoencephalitis, pulmonary tuberculosis, and sinus bradycardia. The patient's symptoms associated with hyponatremia were relatively mild, prompting the need for further evaluation to ascertain the root cause of the electrolyte imbalance. This is particularly important for patients with suspected tuberculosis who are also experiencing complications related to hyponatremia.

Abstract

Chromolaena odorata leaves have been used by the people of Aceh Besar as herbal medicine. The chemical compounds in the leaves of this plant act as antioxidants, anti-inflammatories, antibacterials and wound healers. The research objective was to determine the phytochemical content of C. odorata leaves through qualitative methods. The extraction method used is maceration and testing of bioactive compounds using a color test. The results showed that the yield percentage was 6.05%. The results of the analysis of phytochemical compounds obtained seven phytochemical compounds contained in C. odorata, namely compounds belonging to the class of flavonoids, phenolics, alkaloids, terpenoids, saponins, steroids and tannins. It is recommended to identify chemical compounds using the GC-MS test to obtain specific compounds that play an active role in anti-inflammatory, antioxidant and antibacterial activities. Abstract

Background: Aging occurs in all organs, including the skin. As the largest organ of the body, wrinkles on the skin show signs of aging that are visible and affect appearance. Thread embedding acupuncture (akupunktur tanam benang/ATB) is one of acupuncture methods used in various cases, including aesthetics. This case report describes the role of ATB in the management of nasolabial folds (NLF). Case Illustration: A 45-year-old woman came complaints of deep and long smile lines. On physical examination, the NLF length (measured with digital vernier caliper) right/left: 34.15/30.3 mm; Wrinkle severity rating scale (WSRS) right/left: 3/3. ATB was given once and the results were observed until week 12. The best results were obtained at week 12 of follow-up (NLF: 22.00/21.46 mm; WSRS: 2/2) Conclusion: NLF improvement in ATB is getting better over time. Further clinical trials are needed to determine the effectiveness of ATB in improving NLF. Keywords: Thread embedding acupuncture; Wrinkle; Aesthetic acupuncture

Abstract

Preeclampsia is hypertension that occurs after 20 weeks of gestation with proteinuria. As one of the second highest causes of maternal death in Indonesia, preeclampsia must be detected and managed quickly and appropriately before complications that threaten the mother and fetus arise. One of the conditions that can be associated with preeclampsia is the outcome of low birth weight babies (LBW). This study aimed to determine the relationship between the degree of proteinuria in preeclampsia mothers and incidence of LBW at RSU Karsa Husada Batu. This study used a cross-sectional study design. Data was collected from medical record. The independent variable in this study was the degree of proteinuria and the dependent variable was diagnosis of LBW. The population in this study were all preeclampsia mothers and their babies from preeclampsia patients at RSU Karsa Husada Batu in 2021 with total of 38 patient. Sampling was done by total population sampling. Data analysis used in the study is chi square test. The results of this study showed that there is a relationship between the degree of proteinuria in preeclampsia mothers and incidence of LBW or the higher the degree of proteinuria, the higher the risk of babies from preeclampsia patients experiencing LBW ( p=0,005 ; CI = 95%). Abstract

Thalassemia β major is a hereditary disorder caused by mutations in the β-globin gene, which regulates the formation of one of the components that make up hemoglobin. This disorder results in the production of β-globin chains being reduced or not formed. The prevalence of thalassemia carriers in Indonesia reached 3.8% of the entire population based on data from the Ministry of Health in 2019. Therapy for thalassemia β major patients is lifelong transfusion, which results in iron overload and triggers hypocalcemia and osteoporosis. The aim of this study was to determine the relationship between ferritin levels and calcium levels in thalassemia β major patients. This study uses an observational analytic research type and a cross-sectional research design. This research was conducted at the RSD dr. Soebandi Jember, Jember Clinic Plantation Hospital, and in December 2019-January 2020. The total sample was 13 patients suffering from thalassemia β major aged 1-18 years. The results of the Pearson analysis showed that p=0.858 showed that there was no ignificant correlation between ferritin levels and calcium levels in Thalassemia β major in Jember. Abstract

Hypertension has a high prevalence of patients in the world, as well as in Mumbulsari Jember District. This is due to many risk factors for the incidence of hypertension and doesn’t cause significant symptoms. Psychological stress is one of the factors causing hypertension. Work, economic, and social problems are triggers for stress. This is experienced by farm laborers with complex problems that cause psychological stress conditions. The purpose of this study was to determine whether there is a correlation between stress and increased blood pressure in hypertensive farm laborers in the Mumbulsari Jember Health Center Working Area. This research is an analytic observational research with cross sectional study design. The sampling method was convenience sampling. The data used were primary data (Self Reporting Questionnaire 20 (SRQ-20)) and secondary data (medical records of patients in Puskesmas Mumbulsari Jember). The results of this study showed normal psychology, namely in 11 respondents (42.3%) experiencing stage 1 hypertension conditions and 15 respondents (57.7%) experiencing stage 2 hypertension. SRQ-20 results identified mental distress, namely 23 respondents (60.5%) experienced stage 1 hypertension and 15 respondents (39.5%) experienced stage 2 hypertension. The results of the analysis showed there was no significant correlation between stress and increased blood pressure in hypertensive farm laborers in the working area of the Mumbulsari Jember Health Center with a ρ-value of 0.151 (ρ>0.05). This study concluded that there is no correlation between stress and increased blood pressure in hypertensive farm laborers in the Mumbulsari Jember Health Center Working Area. Abstract

Tuberculosis (TB) and malnutrition are still major public health problems in the world. These two clinical conditions can occur simultaneously and create a vicious cycle that worsens TB infection or nutritional status. This case report aims to examine the relationship between low nutritional status (malnutrition) of patients and death due to pulmonary TB. A 31 year old woman came to the pulmonary clinic of a tertiary hospital in Jakarta for her third month of drug-sensitive anti-TB drug (OAT) treatment control. Three months earlier, the patient underwent a rapid diagnostic test (RDT) to determine OAT resistance. The conclusion of her anthropometric examination was body mass index (BMI) of 15.6 kg/m2. Before being diagnosed with pulmonary TB, she always had difficulty gaining weight. During the course of disease, her weight decreased by 5 kg per month. After undergoing treatment for 6 months, the patient was declared complete after 6 months of therapy, without any improvement in nutritional status. At 1 year follow-up, the patient experienced a relapse of pulmonary TB with a BMI of 13.2 kg/m2. She died 2 months after diagnosis of relapse TB. Improvement in clinical complaints and conversion to bacteriological examination are indications of successful therapy, including improvement in nutritional status. To analyze the relationship between malnutrition and pulmonary TB and death, the authors conducted a comprehensive literature search in the journal databases PubMed, Cochrane, and Scopus, followed by analysis according to the PRISMA 2020 flowchart, for a prognostic cohort study of cases of death in pulmonary TB patients with malnutrition. There is a relationship between BMI and the risk of death in TB patients. Anthropometric measurements, especially BMI, need to be carried out during every physical examination of pulmonary TB cases. Abstract

Yellow wood (Arcangelisia flava) is a medicinal plant that has received little attention. Merr and (L.). Yellow wood has antibacterial, antioxidant, anti-hyperlipidemic, and anti-cancer properties. Furthermore, yellow wood is known to contain a range of flavonoid compounds, such as hydroxylarcangelicin, which has broad antibacterial activity. The purpose of this research is to investigate the total flavonoid content of yellowwood extract as well as its antibacterial activity against Staphylococcus aureus bacteria. The manufacturing of methanol extract, characterization of simplisia, phytochemical screening, and determination of the total flavonoid levels of the yellow wood methanol extract by a visible spectroscopic approach were all part of this stage of research. Next, use chloramphenicol as a positive control and dimethyl sulfoxide (DMSO) as a negative control to test antibacterial activity against Staphylococcus aureus. The results of this study can be concluded to show that the total flavonoid levels in the methanol extract of yellow wood at a metanol concentration of 80% were 228,314 ± 5,4171 mg QE/g. At a 90% concentration of 245.993 ± 6.7626 mg QE/g and a 100% concentration of 265.953 3.0853 mg QE/g. Keywords: Yellow wood; Flavonoids; Spektrofotometri Visible; Stphylococcus aureus;

Abstract

When dealing with clinical disorders known as difficult airways, airway management during general anesthesia still presents challenges. A cross-sectional study done in Indonesia in 2016 found that 28 out of 250 patients receiving general anesthesia had respiratory problems. The existing algorithm must be properly and effectively managed in order to resolve this issue. This case report discusses a 14-month-old pediatric patient with right preauricular lymphoma who will undergo tumor biopsy surgery using general anesthesia. The size of the tumor mass that has grown and covered the patient's airway makes it difficult to manage the airway during general anesthesia, making intubation impossible. To overcome this, bagging was carried out by improvising the technique of reversing the position of the mask cover on the patient during the operation. The patient can be saved and allowed to go home after treatment. Appropriate management during perioperative especially related to airway management will produce good results for patient health and safety. With this case report, hopefully it can add knowledge and an alternative ventilation technique if you encounter a similar case. Keywords: difficult airway, general anesthesia, lymphoma preauricular
